Flu Vaccine Available at Caraluzzi’s

Newtown – Monday, October 24th

11:30 AM – 2:00 PM

 

Georgetown – Wednesday, October 26th

11:30 AM – 2:00 PM

 

Bethel – Friday, October 28th

11:30 AM – 2:00 PM
Flu vaccines will be available for ages 6 months and older.  Preservative-free, Intranasal FluMist (live) vaccines for ages 2 – 49 years, Quadrivalent as well as traditional flu shots will be available.

 

Please Bring your Medical Insurance Card

Bethel VNA can submit claims to the following insurance providers
Aetna, Anthem BC/BS, ConnectiCare, Healthy CT, Medicare Part B & Oxford, Medicare Advantage Plans with Aetna, & Wellcare

 

Please bring your insurance card and Bethel VNA will bill these companies directly. Bethel VNA will bill patient for any unpaid charges. If your insurance company is not listed above, you may pay cash or check. You will get a receipt to submit for reimbursement. Check with your insurance company to be sure the flu vaccine is a covered service. Bethel VNA is NOT a preferred provider for United Health Care or Cigna.

 

Cost

NO charge for any child 4 and under
No charge for children 18 years old or younger who are Medicaid or Husky A/B enrolled, has no health insurance/self pay, American Indian or Alaskan Native, or under-insured

 

If you do not have a participating insurance plan, payments will be accepted by cash, check, health savings account or credit card.  Bethel VNA is NOT a preferred provider for flu vaccines for Cigna or United Health Care (including Secure Horizons).  For more detailed information about Seasonal Influenza vaccines & listing of other local flu clinics, please visit: Bethelvna.org or CDC.gov
